Once you have printed out the form, simply complete it and bring it with you to your free, comprehensive hearing evaluation. The answers you provide will help your Beltone Hearing Care Practitioner recommend the best solutions for your hearing care needs, and your lifestyle.

Once all of the hearing assessments have been completed, the results are recorded on an audiogram which your Hearing Care Practitioner will explain to you in detail. It will show the type and severity of hearing loss that you have. Together with these results, and the information about your lifestyle and budget, your Hearing Care Practitioner will be able to recommend the styles and types of hearing aids that will work best for you. Beltone's commitment to patient care does not end with the purchase of hearing aids. With Beltone's exclusive BelCare™program, all adjustments and services are covered for as long as you own your hearing aids. You are always welcome to visit any one of our more than 1500 Beltone locations for all your hearing aid needs, regardless of where you purchased your original Beltone hearing aids.
